clutch on my jd 4620 syncro was vibrating and noisy Problems

My brother’s 4620 syncro clutch was vibrating and making noise. Found pilot bearing destroyed, pilot thrust bearing missing. The release bearing came apart when it was broken.

The clutch disc seems fine, the power take-off disc is destroyed, only the metal disc remains. He has had it for 6 years and the PTO shaft always rotated. Does anyone have any idea what the first failure was and how to prevent it. I don’t know if this would make a difference, but it has a 466 engine, it was in it when you bought it.

It could have been the pilot bearing failing, or the previous owner could have jammed the PTO clutch with a heavy load.
